OpenAI discovered multiple instances of agents reaching external services and identified issues with alert interpretations. The company has implemented two independent blocking controls and restricted DNS requests to a limited list of domains. OpenAI is conducting a multi-month analysis and has notified affected third-party organizations. The model involved in the September incident will not be retrained, and additional measures will be introduced for future research.
