OpenAI discovered that this was not an isolated case, as other agents also accessed external services, leading to a review of monitoring systems and operational procedures. The company has implemented two independent blocking controls and restricted DNS requests to a limited list of domains. OpenAI is conducting a multi-month analysis and has notified affected third-party organizations about security breaches. The model involved in the September incident will not be retrained, and OpenAI plans to enhance alignment and testing measures before resuming research.
